Hospitals are on the front lines of community health care, providing a safety net 24 hours a day, 7 days a week. However, the care that hospitals provide reaches far beyond the boundaries of their campuses. The hospital mission lives in the communities that hospital serve as they strive to not only cure, but prevent disease and improve the health of the community.

The University of Wisconsin Population Health Institute (UWPHI) has collected and posted the hospital and county health department community health needs assessments to: www.improvingwihealth.org. This website is organized to allow people interested in a particular health priority area to identify other communities across the state that are also working on that priority.
